ABSTRACT

Aims:To measure free hemoglobin percentage in stored packed red blood cells units by colorimetric method.Study Design:This is a descriptive, hospital based case study.Place and Duration of Study:Khartoum central blood bank during the period from June to September 2021.Methodology:A total of thirty-six packed red blood cells blood bags with Citrate-Phosphate-Dextrose-Adenine-one (CPDA-1) stored at 2-6°C for 35 days were withdrawn aseptically on days one; 14, and day 35. Free hemoglobin concentration was measured with Drabkin's method through digital photoelectric colorimeter in which the percentage of hemolysis was calculated, and PH values were measured by a PH meter (ADWA). Mean and standard deviation ofeach day were calculated by statistical package for social science (SPSS)computer program version 22.0.Results:The results showed statistically significant elevation in percentages of free plasma hemoglobin with prolongation of storage duration periods; day one compared with day 14 (P= 0.001), day one compared with day 35(P= 0.001) and day 14 compared with day 35 (P= 0.001). No significant correlation was observed between the degree of hemolysis in day 35 with PH values (P= 0.9).Conclusion:This study concluded that prolonged storage is associated with elevation of free plasma hemoglobin level indicative of progressive hemolysis.

ABSTRACT

Cell-free hemoglobin refers to the rupture of erythrocyte membrane due to instability of erythrocyte membrane caused by various causes, and hemoglobin is released free in the plasma.High concentration of plasma cell-free hemoglobin can cause intracellulariron ion deposition, oxidative damage, damage of lung, kidney and other related tissue, and seriously affect the treatment and prognosis of patients with sepsis.Nuclear factor E2-related factor 2(NRF2) as an antioxidant cytokine at the same time, plays important roles in the process of adjusting the resistance to oxidative stress, but the mechanism is not thorough.This review summarized the sepsis cell-free hemoglobin production, metabolism, and the activation of NRF2 and antioxidant mechanisms, to provide reference for further research.

ABSTRACT

Although some investigations have been performed to determine the effects of transfusion load and suction pressure on renal function during intraoperative salvage autotransfusion, the precise threshold is still undetermined. A total of 625 patients undergoing surgery with the Continuous AutoTransfusion System (CATSplus) were enrolled and divided into groups according to the utilized suction pressure and transfusion volume. Plasma free hemoglobin (FHB) and creatinine clearance (CCr) were assayed to indicate the renal function. Both 0.03 MPa suction (≥4-unit load) and >5 units transfusion changed the levels of FHB and CCr significantly when measured 24 h post-operation compared to pre-operation. Under 0.02 MPa suction (≥4-unit load), the alteration of FHB and CCr returned to normal after 24 h. Under 3 units transfusion, the levels of FHB and CCr at 6 and 12 h post-operation changed significantly compared to pre-operation (P<0.05 or P<0.01, respectively), and this alteration could be restored to normal at 72 h post-operation. After an exhaustive investigation, less than 4 units transfusion and less than 0.03 MPa suction pressure are recommended for intraoperative salvage autotransfusion.

ABSTRACT

【Objective】 To compare the similarities and differences of polymerized human cord hemoglobin (PolyCHb) and free hemoglobin (FHb) on partial coagulation indexes in vitro, so as to analyze the effect of PolyCHb on coagulation dysfunction. 【Methods】 Using normal saline, two concentrations of FHb and PolyCHb and 36% methemoglobin-containing PolyCHb to mix with fresh whole blood or plasma-rich plasma (PRP) in equal proportions, and incubate at 37°C for 30 minutes to detect prothrombin time (PT), activated partial thromboplastin time (APTT), coagulation factor Ⅱactivity (FⅡ∶C), coagulation factor Ⅴactivity (FⅤ∶C), coagulation factor Ⅷactivity (FⅧ∶C), coagulation factor Ⅸactivity (FⅨ∶C), von Willebrand factor (vWF) and platelet P-selectin (CD62P). 【Results】 1) NaCl group: PT(22.68±1.76) s; APTT(59.58±7.52) s; FⅡ∶C(45.91±3.27) %; FⅤ∶C(30.86±4.43) %; FⅧ∶C(41.32±12.94) %; FⅨ∶C(23.96±5.10) %; vWF (2.14±0.54) mg/L; CD62P(7.44±4.47) %. This group kept as a diluted control. 2) 2% FHb group compared with 7% FHb group: FⅧ∶C (42.16±12.31) %vs (56.64±12.22 ) % (P0.05). 3) There is no significant difference between 2% PolyCHb group and 7% PolyCHb group (P>0.05). 4) There is no significant difference between 2% FHb group and 2% PolyCHb group (P>0.05). 5) 7% FHb group compared with 7% PolyCHb group: PT(23.31±1.34)s vs (21.97±1.56)s (P0.05). 6) 7% PolyCHb group Compared with Met-PolyCHb group: APTT(55.43±5.43) s vs (46.33±4.86)s (P0.05). 【Conclusion】 The effect of PolyCHb on coagulation markers is different from FHb. At the concentration of this study, PolyCHb will not cause coagulation disorders. However, if the methemoglobin (MetHb) content is too high, it will activate the intrinsic coagulation pathway.

ABSTRACT

Objective To establish the test way of latex enhanced immune turbidimetric method for the concentration detection of urine free hemoglobin on the automatic biochemical analyzer,and evaluated the detection performance.Methods Prepared the detection reagents of latex enhanced immune turbidimetry method for detection of urine free hemoglobin with materials such as hemoglobin HBA0 antibody (8.5 mg/ml),80 nm carboxylic olystyrene latex microspheres (estapor,nanometer microspheres) and so on.Then,seted the project parameters on TBA120 automatic biochemical analyzer and proceeded the calibrations.Evaluated the detection performance by accuracy,precision,linear range,sensitivity,specificity and other indicators.Lastly,established the biological reference interval for urine free hemoglobin of healthy people in the region and verified it.Results To seted the project parameters on TBA120 automatic biochemical analyzer and passed the calibration.The calibration fitting curve had a smooth trend and no obvious deviation with each measuring point.In the recovery tests,added standard liquid with different concentrations of 100 mg/L and 500 mg/L to the samples with low values,the recovery rate was 95.3% and 102.7% respectively.Same to the samples with high values,the recovery rate was 104.2% and 103.5% respectively.In the precision test,samples with low and high value had a precision CV of 6.52% and 4.18% respectively.The linear range was 0 ~ 1 100 mg/L,analysis sensitivity was 2.8 mg/L.In the interference test,found that,samples had no obvious disturbance to the test with lower concentrations of free bilirubin,combined with bilirubin,vitamin C,animal hemoglobin lower than 342 μmol/L,342 μmol/L,0.03 g/L,500 mg/L respectively,while had significant interference when the concentration of chyle was higher than 870 FTU.Their study had established the biological reference interval of healthy people,male was 0~ 13.3 mg/L and female 0 ~ 17.1 mg/L,there was no significant difference between them by a t test (P>0.05).Conclusion The latex enhanced immune turbidimetric method for the detection of urine free hemoglobin has the performance of testing clinical urine specimen,which not only solved the lackage problem of specificity for occult blood in the urine dry chemistry test and realized batch automation quantitative detection.The experimental data of this study provides a theoretical basis for the application of the method in other types of clinical samples.

ABSTRACT

Objective To comprehensively analyze the effect and various detection indicators of leucocyte depletion suspended red blood cells(RBC) prepared by different storage time and different manufacturers of leucocyte depletion instrument to provide some decision-making basis for peers .Methods Two hundreds and forty bags of suspended RBC(1 ,1 .5 ,2 U) prepared from whole blood by 4 manufacturers were randomly collected and preserved in (4 ± 2)℃ dedicated refrigerator according to different specifica-tions .Then the sterile quick-release connector was used to take 240 leucocyte depletion suspended RBC samples of same specifica-tions and different shelf lives of 0 ,7 ,14 ,21 ,28 ,35 d by different manufacturers .Their capacity ,hematocrit ,leukocyte residues ,plas-ma free hemoglobin content and hemoglobin content were detected ,and the hemolysis rate at the end of shelf life was calculated .Re-sults The effect of leucocyte depletion suspended RBC by the same manufacturer had no statistical difference among different spec-ifications (P>0 .05);the capacity and plasma free hemoglobin content in the same specification had statistical difference among difference among different manufacturers (P<0 .05);the free hemoglobin and hemolysis rate at the end of shelf life in the same manufacturer had statistical difference among different preserving time (P<0 .05) .Conclusion By comprehensive considering the results of indexes of capacity ,hematocrit ,filter time ,free hemoglobin content and hemolysis rate ,our station chose the Shandong Weigao and Fresenius disposable leucocyte depletion plastic bags for ensuring the leucocyte depletion effect ,and safety and efficacy of RBC transfusion .

ABSTRACT

Objective To study the effect of vibration on free hemoglobin(FHb), lactic dehydrogenase(LDH), and K+concentration of red blood cells(RBCs)stored for different lengths of time.Methods Thirty-five bags of RBCs stored for different lengths of time(3,7,11,26,and 35 d,7 bags per group)were chosen as the target of research.A wheeled vehicle that could vibrate in three directions(horizontal,longitudinal,and vertical)was used to simulate RBC transportation in addition to an electromagnetic vibration test system.The RBC samples were prepared and vibrated for 0 h,or 3 h for detection and analysis of their FHb, LDH, and K+concentration.Results The FHb concentration was significantly increased after vibration(P<0.05), and tended to increase with the extension of storage time.LDH(P<0.05)and K +concentrations(P<0.05)were obviously increased with the length of storage(3,7,11,26,and 35 d), and significantly increased after vibration of 3 h.Conclusion FHb concentration is increased after vibration, and the storage time could change FHb.The vibration and storage time have great effect on LDH and K +concentrations.

ABSTRACT

Objective To study the thermal insulation properties of vacuum insulated panel(VIP)used as materials for blood transportation kits.Methods A VIP transportation kit was taken as test group,and a conventional transportation kit was taken as control.Phase change cool storage materials of the same amount and 4℃water bags were put into both kits.A recordable electronic thermometer was used to record the temperature within the two kits,and free hemoglobin(FHb)and potassium ion concentration were measured at 0, 4, and 7 days after blood storage.Results The temperature in the conventional transportation kit increased faster after 36 h, and was significantly higher than in the VIP transportation kit until 60 h.Besides,the VIP transportation kit kept the temperature under 10℃ for(60.7 ±0.6)h, compared to (54.2 ±3.0)h in the conventional transportation kit.FHb concentration was significantly lower in the VIP transportation kit[(605.26 ±74.63)mg/L]than in the conventional transportation kit[(1327.60 ±187.41)mg/L]at day 7(d 7),so was the potassium ion concentration in the VIP transportation kit[(22.7 ±0.4)mmol/L]compared to[(24.6 ±0.6) mmol/L]in the transportation kit at d 7.Conclusion The VIP transportation kit keeps the temperature under 10℃ for a longer time,and the blood quality of preservation is better than that of the conventional transportation kit.Novel heat preservation material can improve health support ability,and is of great value and significance.

Objective To prepare an internal quality control substance of free hemoglobin (FHb),and evaluate its perform-ance.Methods ① 5 normal whole human blood samples with known hemoglobin (Hb)concentration was selected and dilu-ted with distilled water in the ratio of 1∶500,1∶1 000,1∶2 000,1∶4 000 respectively.After 20 FHb solutions was fully mixed,the concentration of FHb solutions was tested and compared with its theoretical data.②One of the highest value and the lowest value solutions were chosen as qualitye control substance,packed and stored in -20℃ fridge.Each group was an-alyzed 20 days continuously in the routine conditions,where average (-x )of the groups,standards deviation (s )and coeffi-cients of variation (CV)were calculated and Levey-Jennings control chart was conducted.Then,the test was repeated every week,and the data on the chart was updated accordingly until (CV)of the reagents were changed.The average (-x)of the-groups,standards deviation (s)and coefficients of variation uality control material was calculated during the whole test peri-od.Results ①The theoretical value (-x ±s)of the concentration of free hemoglobin was 125.44±93.04 mg/L,and the actu-al value was 125.22±93.08 mg/L,there was no statistic significance (t value was 0.706,P >0.05).② During 20 continu-ously daystest,the detected values (-x ±s)of the highest quality control material was 303.55±3.70 mg/L;the values of the lowest quality control material was 69.29 ± 1.88 mg/L.The coefficients were 1.22% and 2.68% respectively.Both the highest and the lowest quality control material were under control during the whole detecting period.The (-x ±s )values of the highest was 302.56±3.99 mg/L,the CV value of it was 1.32%;The (-x ±s)values of the lowest was 69.04±1.88 mg/L,the CV value of it was 2.71%.Conclusion The preparation method of internal quality control substance of free hemoglo-bin is convenient and stable,and was applicable in blood station’s and clinical laboratory.

ABSTRACT

Objective To observe the effect of nitrous oxide (N2O) on the content of serum free hemoglobin ,and intercellular adhesion molecule‐1 (ICAM‐1) of patients with HIFU Therapy ,and investigate its action of tissue damage mechanism .Methods 50 patients with primary liver cancer undergoing HIFU surgery (ASA Ⅰ - Ⅱ class) were randomly divided into control group (group C) and experimental group(group N) ,25 patients of each group .General anesthesia method was used in both two groups , group C was by total intravenous anesthesia ,group N was adopted intravenous‐inhalation anesthesia .both two groups was adopted the same anesthesia induction method .anesthesia maintain of group N was joined N2 O on the basis of group C .both two groups were draw blood from the radial artery at the points of before anesthesia (T1 ) ,before operation (T2 ) ,1 h (T3 ) ,2 h (T4 ) ,3 h (T5 ) after intraoperative ,and 24 h after operation (T6 ) ,peroxidase reaction test and double antibody sandwich ELISA method were a‐dopted to detect the content of Fhb value and ICAM‐1 ;ultrasonography system of HIFU therapeutic instrument was used to meas‐ure the abdominal wall thickness of patients before and after operation .Results The content of FHb and ICAM‐1 in serum were significantly increased after operation than before with the anesthesia time (P<0 .05);compared with group C ,group N increased obviously at the same point in time (P<0 .05);preoperative and postoperative abdominal wall thickness value of group N was in‐creased significantly (P< 0 .05) .Conclusion It may be connected with N2 O enhanced ultrasound cavitation effect that the body produces more FHb and ICAM‐1 of group N in HIFU treatment ,and induces abdominal wall skin markedly swollen .

ABSTRACT

Background: Gamma irradiation has been often used for inactivating viable T lymphocytes in the blood product to prevent post-transfusion graft - vs - host disease (PT - GVHD) occurred in the recipients having immunodeficiency. But the irradiating method must be determined to minimize the reverse effects on other blood elements. Objectives: to evaluate the side effects of gamma irradiating with 25 Gy and in different ways on red cells in stored red cell concentrates. Subjects and methods: Each of 30 red cell concentrates was divided into 3 parts and treated in 3 different ways to create 3 groups of sample for studying. All the samples were stored 35 days at 4 - 6\xb0C and measured red cell osmotic fragility, hemoglobin (Hb) level in storage solution in days 1,7,14,21,28,35. Results: 1) Red cell osmotic fragility decreased and free Hb level increased with time in the samples of 3 groups. 2) Red cell osmotic fragility decreased strongly from the 28th day (p < 0.05) and free Hb level increased dramatically from the 14th day (p < 0.001) in the samples of group 2 compared to control group and group 3 with the same storage time, while there was no difference in these parameters in groups 1 and 3. Conclusion: 1) Gamma irradiation with 25 Gy affected the intact of red cells in red cell concentrates only when irradiated right after being produced and stored more than 14 days. 2) The quality of red cell would be unchanged if used immediately after irradiated. \r\n', u'\r\n', u'

ABSTRACT

BACKGROUND: Intraoperative autotransfusion or residual blood in a cardiopulmonary bypass (CPB) circuit has been used to reduce the need for an homologous blood transfusion during cardiac surgery. However, it may contain some free hemoglobin released from damaged cells. The load of blood containing free hemoglobin may cause renal dysfunction. We measured the amount of free hemoglobin in banked blood, cell saver blood and CPB blood to evaluate what is the least hemolytic blood transfused in cardiac surgery. METHODS: This study was performed in 20 patients undergoing cardiac surgery. In each patient, the banked blood, intraoperative salvaged blood with a cell saver and CPB residual blood were sampled at the end of the operation. The concentration of free hemoglobin, hemoglobin and platelet counts were measured in these blood samples and percent of hemolysis was calculated (%hemolysis = [free hemoglobin] / [free hemoglobin hemoglobin]) RESULTS: In salvaged blood with a cell saver, CPB residual blood and banked blood, hemoglobin concentrations were 20.1+/-2.7 g/dl, 8.0+/-1.1 g/dl, and 22.2+/-2.7 g/dl, respectively. Free hemoglobin concentrations were 336.6+/-239.5 mg/dl, 49.2+/-26.8 mg/dl, and 279.5+/-167.5 mg/dl respectively. Platelet counts were 26.1+/-22.2X10(3)/mm3, 116.8+/-56.5X10(3)/mm3, and 94.9+/-43.6X10(3)/mm3 respectively. % Hemolysis were 1.6+/-1.1%, 0.6+/-0.4%, and 1.2+/-0.7% respectively. In the comparison between the salvaged blood with a cell saver and CPB residual blood, free hemoglobin concentration, % hemolysis, and platelet counts had positive correlations (r = 0.8, 0.7, and 0.6). CONCLUSIONS: In twenty cardiac surgeries, CPB residual blood had a lower free hemoglobin level than the other two blood groups. The platelet counts in CPB residual blood were higher than those in cell saver blood but did not differ from those in banked blood. Therefore, CPB residual blood was the least hemolytic blood among the three blood groups when a transfusion was performed in cardiac surgery.

AIM:To investigate the influence of free hemoglobin on the initiation and progression of atherosclerosis and the role of CD163 in this process.METHODS:Thirty male SD rats were randomly divided into three groups:control group(group C),atherosclerosis group(group A),atherosclerosis and hemolysis group(group P).The hemolysis and atherosclerosis animal model was established.The free hemoglobin(FHb)and MDA levels in plasma,(RAAPIs)and intima area/midmembrane area(I/M)of each group were measured.The expressions of CD163 and heme oxygenase-1(HO-1)in atherosclerosis plaques in group A and P were detected and measured by means of immunohistochemistry and Western blotting.RESULTS:Compared with group C,the FHb,MDA,CD163 and HO-1 in group A and group P increased significantly(P0.05).The FHb level in plasma and the expressions of CD163 and HO-1 in atherosclerotic plaques correlated with each other(r=0.526,r=0.498,r=0.653;P

ABSTRACT

Objective:To explore the change of membrane protein composition of red blood cells after the blood being filtered by leukocyte filter.Methods:A total of 400ml blood from 20 healthy donors were equally divided into control group and filtered group according to the paired design.SDS-PAGE electrophoresis was done for the red blood cell membrane protein of both groups.Bandscan protein analysis software was used to analyze the composition of membrane protein.Fluorescence degree of polarization was detected by fluorescence polarization method and membrane flowability was counted.Plasma concentration of free hemoglobin(FHb) was detected weekly.Results:The band Ⅰ,Ⅱ and Ⅶ parotein of membrane skeleton of the filtered group decreased significantly compared with those of the control group(P﹤0.01),and the fluorescence degree of polarization and microviscosity of the filtered group was higher(P﹤0.01).There was significant difference of plasma FHb between filtered group and control group at each week except week 0,and that of the filted group was higher(P﹤0.01).Conclusion: Leukocyte filter can bring damage to erythrocyte membrane protein composition.Membrane microviscosity and plasma concentration of free hemoglobin increase and membrane flowability decreases.
